Then there was the insulin-resistant adult-onset version. Today, there are at least 6 different diabetes subsets. That’s staggering. They include: Type 1. 					<description><![CDATA[<p>University of Illinois Chicago via Newswise &#8211; Researchers at University of Illinois Chicago have identified changes in the gut microbiome that can lead to gestational diabetes.  “The bacteria that live in your gut impact your risk for diseases like gestational diabetes. How they do it, or lead to those outcomes, is not clear. We performed a study to identify those factors in gut bacteria that are predisposing people to some of these diseases,” said Dr. Brian Layden, associate professor and division chief for the UIC College of Medicine, endocrinology/diabetes and metabolism department, and co-author of the study.  They studied the exact nature of the gut microbiome and plasma metabolome changes throughout pregnancy in mice. To aid in understanding changes to the gut microbiome during pregnancy that affect metabolism, researchers analyzed mice that received transplanted human fecal samples.   The mouse models were used to identify the role of indoleamine-2,3-dioxygenase, or IDO1, in pregnancy-associated insulin resistance, according to the study. Insulin resistance is a feature of Type 2 diabetes and can be present in prediabetes. Through this approach, researchers identified a metabolite, kynurenine, that is increased during pregnancy.   “We identified a particular way that the bacteria interact with gut epithelial cells and regulate how a metabolite is generated which impacts somebody’s risk for something like gestational diabetes,” Layden said. The paper, “Gestational insulin resistance is mediated by the gut microbiome-indoleamine 2,3-dioxygenase axis,” was published recently in the journal Gastroenterology.   Gut microbiome changes during pregnancy, shifting ID01-dependent metabolism toward kynurenine production, intestinal inflammation and gestational insulin resistance. This process can be reversed when ID01 is inhibited, according to the study.  The implication of these findings is that modifying or changing the gut bacteria so it does not lead to more of this metabolite could minimize the development of a diabetes-like state in pregnancy, Layden said.  “These findings open up the opportunity for a screening approach for those who are at risk as well as a treatment approach for gestational diabetes where if you know they are at risk, we can modify their gut bacteria to prevent it from occurring,” Layden said. Screening could potentially be done prior to pregnancy.  In the United States, 1-2% of pregnant people have Type 2 diabetes and about 6-9% of pregnant people develop gestational diabetes, according to the Centers for Disease Control. Gestational diabetes puts both the pregnant person and baby at risk for complications during pregnancy and childbirth, as well as increasing the risk of developing Type 2 diabetes after delivery for both the pregnant person and the baby (later in life).   Currently, gestational diabetes is treated through diet and exercise, or medication in more severe cases. Layden said there are existing trials researching whether prebiotics and probiotics are beneficial for Type 2 diabetes and gestational diabetes. However, this new study could lead to more targeted approaches to gut microbiome health.  Researchers plan to conduct screenings for these metabolites in human fecal and blood samples to see if the research translates to humans. 					<description><![CDATA[<p>Rutgers University-New Brunswick via Newswise &#8211; Exposure to phthalates—a group of chemicals found in everything from plastics to personal care products to electronics—may disrupt an important hormone needed to sustain a healthy pregnancy, according to a Rutgers study. The study, which appears in Environment International, was among the first to examine the impact that phthalates, added to plastics to increase flexibility, have on the placental corticotropin releasing hormone (pCRH) that is produced by the placenta and increases throughout the course of pregnancy. The hormone plays an important role in promoting the onset of labor, but when levels are high or rise rapidly earlier in pregnancy, it may contribute to preterm birth and fetal growth problems as well as high blood pressure, diabetes, and postpartum depression. “We are all exposed to phthalates in our environment through the products we use and the foods we eat,” says Emily S. Barrett, an associate professor at the Rutgers School of Public Health and member of the Environmental and Occupational Health Sciences Institute. “Our findings show that these chemicals may alter the production of essential placental hormones, which has important implications for the course of pregnancy as well as subsequent child health and development.” The researchers analyzed data from 1,018 low-risk pregnant women carrying single fetuses at two time points, mid- and late pregnancy. They found that the presence of various phthalates was associated with higher pCRH hormone levels in mid-pregnancy, but lower pCRH later in pregnancy. These levels were strongest in women who developed pregnancy complications like gestational diabetes and high blood pressure, suggesting that women who develop complications may be particularly vulnerable to this hormonal disruption. This is among the first and largest study so far to examine how these chemicals and the connection with pCRH disrupt the function of the endocrine system, which is especially delicate during pregnancy. “Associations between phthalates and pCRH among women with pregnancy complications grew stronger across the course of pregnancy. We know very little about how women with pregnancy complications are affected by environmental exposures. This study sets the stage for future research in that area,” Barrett said. pCRH produced by the placenta is identical in structure to corticotropin-releasing hormone (CRH) produced by the brain when responding to stress. While this study did not find that women who had experienced childhood traumas were any more vulnerable to the hormone-disrupting effects of phthalates, earlier research found that pCRH levels were higher in women who have experienced childhood trauma suggesting that traumatic events may impact pregnancy decades later. 					<description><![CDATA[<p>Flinders University via News-Medical Net &#8211; Healthy dietary choices are not only important for expectant mothers during the early months of pregnancy but equally important in the months prior to conception. Fast food consumption prior to pregnancy was found to be a risk factor for gestational diabetes, while low consumption of green leafy vegetables and fruit was found to increase the risk for several pregnancy complications including gestational diabetes, spontaneous pre-term birth and having a small for gestational age baby. Researchers at Flinders University’s College of Medicine and Public Health have studied genetic and lifestyle factors that could affect pregnancy health and outcome. The study, led by a world-renowned placental biologist and pregnancy expert Professor Claire Roberts, involved 3196 women with a first-time pregnancy, recruited into the SCOPE Study in Adelaide and Auckland between 2005 and 2008. Professor Roberts’ team found that both maternal and paternal genes involved in folate and folic acid metabolism influenced pregnancy outcomes, including preeclampsia, gestational hypertension and spontaneous pre-term birth. While there is not much that can be done with the genetic hand women are dealt, there are modifiable lifestyle factors, such as diet, that women can have control over when it comes to their pregnancy. We know from our previous research that mothers with folate deficiency are more likely to have a number of pregnancy complications, and this study supports the benefits of increasing folate levels via green vegetable consumption to support pregnancy health.” Claire Roberts, Professor, Flinders University Women who are planning to conceive are recommended to take folic acid supplementation at least one month before conception and during the first few months of pregnancy to minimize the potential for folate deficiency and reduce the risk of having a baby with a neural tube defect. However, in this study, conducted before the introduction of folic acid fortification mandate in Australia, women who developed gestational hypertension and gestational diabetes mellitushad higher serum folate levels at 15 weeks’ gestation compared to women who had an uncomplicated pregnancy. Researchers say further studies are needed to assess the relevance of the association between maternal circulating folate levels and gestational hypertension and potentially gestational diabetes, particularly in light of the mandatory folic acid fortification program. Interestingly, this has coincided with a steep rise in the incidence of gestational diabetes in Australia. In South Australia, gestational diabetes was at 5.5% in 2009 but by 2017 it had risen to 14%. The Australian government’s 2009 decision to implement mandatory folic acid fortification of flour for commercial bread-making and pastries has resulted in increased folic acid consumption by the general Australian public. As a result, pregnant Australian women are now consuming higher levels of folic acid through a combination of fortified diets and micronutrient supplementation. Due to this shift, Professor Roberts’ Pregnancy Health and Beyond research team is now studying the effects of too much folic acid can have during pregnancy and is trying to determine what effects this will have on not only pregnancy health, but also the health of the mother and her baby later in life. From our current research, we know that both serum and red blood cell folate levels in pregnant women recruited in Adelaide within the past few years are up to 4 times higher than those measured in women from this study. 					<description><![CDATA[<p>Naama Barak via Israel21c &#8211; Weizmann Institute scientists discover nine parameters that identify women at a high risk of developing diabetes during pregnancy. Pregnancy is a time full of wonder and excitement, but also of concern and what seems like never-ending and strange medical tests. A particularly annoying one is the glucose tolerance test, which involves drinking what tastes like pure sugar to rule out gestational diabetes. This pleasure, however, may be replaced by a far less sickly nine-question questionnaire, courtesy of a recently developed Israeli algorithm that can predict the risk of gestational diabetes even before a woman becomes pregnant. This is good news as gestational diabetes, which is characterized by high blood-sugar levels that develop during pregnancy in women who did not previously have diabetes, can be dangerous for both mother and child. It occurs in three to nine percent of pregnancies and is usually diagnosed between the 24th and 28th weeks of pregnancy. The algorithm was created by scientists from the Weizmann Institute of Science, who recently shared their findings in the Nature Medical journal. Led by Prof. Eran Segal, the researchers analyzed data on nearly 600,000 pregnancies available from Israeli HMO Clalit Health Services.They applied a machine-learning method to health records on some 450,000 pregnancies in women who gave birth between 2010 and 2017, 4% of whom had been diagnosed with gestational diabetes. After processing the data, the algorithm revealed that nine parameters can accurately identify the women at a high risk of developing gestational diabetes. These include the woman’s age, body mass index, family history of diabetes and results of her glucose tests during previous pregnancies, if any. To validate their findings, the researchers then applied the nine parameters to health records on an additional 140,000 pregnancies that had not been part of the initial analysis, proving them to be correct.
