Understanding the cause of acid reflux Acid reflux is caused by the incomplete closure of the sphincter valve – located between the stomach and the esophagus. When this occurs, harsh stomach contents can splash back up across the delicate lining of the esophagus and the throat, nose, and other vulnerable tissues. Most commonly, acid reflux can cause pain in the center of the chest and, at times, an acidic taste in the mouth. While uncomfortable, the true concern is that – left untreated – symptoms of acid reflux will lead to esophageal cancer and other related changes down the road. But one thing should be made perfectly clear: toxic medications will never cure this problem. The dangers of PPI drugs and acid reflux What many acid reflux sufferers don’t realize is that medications to treat their heartburn and other symptoms come at a hefty price. Using PPIs to gain some control over symptoms may seem helpful, but these medications don’t stop the physical reflux of digestive enzymes, bile, and corrosive food/drinks flowing into the delicate esophageal lining. Continued exposure to the acids and protein-digesting enzymes in stomach fluid can bring about precancerous changes, such as the development of the condition known as Barrett’s esophagus. Reducing stomach acid is also likely to interfere with your body’s ability to extract vitamin B12 from foods. Research shows that most oral acid-suppressing medications, including PPIs, decrease the absorption of vitamin B12 from foods. In one study, 75 percent of PPI users were deficient in vitamin B12, compared with just 11 percent among non-users. Gut problems can be life-threatening Regular use of PPIs may also lead to low blood magnesium levels. Symptoms include fatigue, unsteadiness, numbness/tingling, seizures, and heart rhythm disturbances. In severe cases, hospitalization may be necessary. However, magnesium levels can return to normal when PPI medications are halted. PPI use can also lead to poor calcium absorption, increasing the risk of bone fractures. These medications are also associated with elevated risk for cardiovascular disease and even risk of death due to heart attack. PPIs inhibit the normal production of nitric oxide, a signaling molecule vital for normal blood vessel function. Up to 50 percent of sufferers turn to these drugs. With the backing of big pharma, these brand names have become synonymous with relief for many. Currently, around 7% to 15% of the U.S. population, regardless of age or gender, regularly uses proton pump inhibitors (PPIs). These drugs are commonly prescribed to treat reflux and heartburn by reducing stomach acid, and millions more buy them over the counter. However, emerging research links long-term PPI use to potential kidney damage, a serious condition that can develop quietly, often without noticeable symptoms. Major warning about heartburn drugs: The problems can develop “silently” In a five-year study of 125,000 patients published in Kidney International, researchers at Washington University School of Medicine in St. Louis found that more than half of the cases of chronic kidney damage and end-stage renal damage occurred in people without any previous record of kidney problems. According to senior study author Dr. Ziyad Al-Aly, assistant professor of medicine at Washington University School of Medicine, the study showed that kidney problems in patients taking PPIs could develop silently and gradually over time, eroding kidney function and leading to long-term kidney damage or even renal failure. End-stage renal disease is the failure of kidneys to remove waste from the body, necessitating either dialysis or a kidney transplant. Dr. Al-Aly warned that doctors must carefully monitor kidney function in patients taking PPIs – including lansoprazole, omeprazole, and esomeprazole – even when there are no signs or symptoms of damage. The study also evaluated 18,436 new users of another type of heartburn medication: histamine H2 receptor antagonists, or H2 blockers. While H2 blockers don’t work as well on heartburn, researchers found they are less likely to cause kidney problems. Additional studies confirm researchers’ disturbing findings An earlier study published in the Journal of the American Society of Nephrology also showed that long-term use of heartburn medications is associated with kidney damage, with PPI users 28 percent more likely to experience chronic kidney disease and a shocking 96 percent more likely to develop kidney failure. The longer the drugs were taken, the higher the risk. The daily dosage also affects risk. In a study published in JAMA Internal Medicine, the team found that twice-a-day users of PPIs tripled their risk over people who took a single daily dose. The research team called for using PPIs only when “medically necessary” and limiting the duration of the treatment as well. Lead author Dr. Morgan Grams, assistant professor of epidemiology at Johns Hopkins University, pointed out that up to 70 percent of acid reflux prescriptions are handed out “inappropriately” – and he estimated that a full 25 percent of long-term users could stop taking the medication without suffering increased heartburn or acid reflux. Of course, stemming the flow of prescribed PPIs will do nothing to reduce the number of people buying – and taking – these medications over the counter. And, the damage from PPIs isn’t limited to the kidneys. PPIs affect the absorption of essential vitamins and minerals PPIs interfere with the body’s ability to extract vitamin B12, an essential nutrient, from foods. Studies have shown that 75 percent of PPI users are deficient in vitamin B12 – compared to 11 percent among the general non-using population. PPIs can cause dramatic declines in blood magnesium levels, characterized by symptoms of fatigue, unsteadiness, numbness and tingling, seizures, and heart arrhythmias. As if that weren’t enough, PPIs can also lead to poor calcium absorption, raising the risk of bone fracture – especially in older patients. In addition, PPI users are more likely to be obese and have high blood pressure. PPIs are also associated with higher rates of pneumonia and C. difficile, a potentially dangerous bacterial infection. And finally, a German study showed that older adults who take proton pump inhibitors have a 44 percent increased chance of developing dementia. Natural solutions exist for acid reflux Many natural health experts point out that heartburn typically results from too little stomach acid rather than too much – and that PPIs can actually worsen the problem. You may be able to ease heartburn and acid reflux with simple dietary and lifestyle changes – such as avoiding fried, fatty, and processed foods, eating smaller meals, chewing food well, and avoiding smoking and excessive drinking. Eating healthy amounts of vegetables and high-quality organic, unprocessed foods can help restore natural gastric balance, while consuming fermented foods – such as sauerkraut, miso soup, and kimchi – can help eliminate the H. pylori bacterium that can contribute to reflux. Natural, time-honored remedies for heartburn include unfiltered apple cider vinegar, baking soda, ginger root tea, chamomile tea, and slippery elm. In the ABCs of health consequences of obesity, G is for Gallstones. The top digestive reason people are hospitalized is a gallbladder attack. Every year, more than a million Americans are diagnosed with gallstones, and about 700,000 must have their gallbladders surgically removed. It is a relatively safe procedure, with complication rates tending to be under 5 percent and a mortality rate of only about 1 in 1,000. However one in ten may develop a post-cholecystectomy syndrome of persistent gastrointestinal symptoms long after their gallbladder is removed. What are gallstones made of? In 80 to 90 percent of cases, gallstones are mostly crystalized cholesterol, forming like rock candy in our gallbladder when cholesterol gets too concentrated. This was used to explain why some small, early studies found that non-vegetarians had a higher incidence of gallstones. However, results from more recent, larger studies are more equivocal; one study suggests that a “vegetarian diet may therefore protect” against gallbladder disease, for instance, while another shows higher rates among vegetarians, independent of weight. As I discuss in my video The Effects of Obesity on Gallstones, Acid Reflux, and Cardiovascular Disease, the biggest purported cause-and-effect risk factor may be obesity, increasing risk as much as sevenfold, as you can see below and at 1:32 in my video, with a doubling of risk even at the heavier side of “the normal BMI range.” Ironically, rapid weight loss may also be a trigger. Half a pound (0.2 kg) a day has been deemed the upper limit for medically safe weight loss, based on gallstone formation. Ultrasound studies found that, above that, the chance of new gallstones can go from less than 1 in 200 a week to closer to 1 in 40, as shown below and at 1:59. To help prevent a gallstone attack, we can increase our fiber intake. Not only is dietary fiber intake associated with less gallbladder disease in the first place, but those placed on high-fiber foods during a weight-loss regimen suffered significantly less gallbladder sludging than those losing the same amount of weight without getting the extra fiber. G is also for gastroesophageal reflux disease, or GERD. Fiber-rich food consumption decreases the risk of acid reflux, too. I previously explored how chronically straining at stool may push part of the stomach up into the chest cavity. Well, excess abdominal pressure due to obesity may have the same effect, pushing acid up into the throat and causing heartburn and inflammation. The increased pressure on the abdominal organs associated with obesity may also explain why overweight women suffer from more vaginal prolapse, where organs such as the rectum push out and into the vaginal cavity. The deadliest letter in the alphabet of obesity consequences is H, which is for Heart Disease. Of the four million deaths attributed to excess body weight every year around the world, nearly 70 percent are due to cardiovascular disease. Is it just because of eating poorly? Mendelian randomization studies suggest that people randomized from conception to be heavier—based only on genetics—do indeed have higher rates of heart disease and stroke regardless of what they eat. The question is: If you lose weight, does your risk drop? Enter the Swedish Obese Subjects (SOS) trial, the first long-term controlled trial to compare the outcomes of thousands of bariatric surgery patients to matched control subjects who started out at the same weight but went the nonsurgical route. Individuals in the control group maintained their weight, whereas those in the surgical group maintained about a 20 percent weight loss over the next 10 to 20 years. In that period, those in the weight-loss group not only developed 80 percent less diabetes, but they suffered significantly fewer heart attacks and strokes. So, unsurprisingly, they significantly reduced their total mortality overall. Ten years out, the weight-loss group appeared to cut their risk of fatal heart attacks and strokes in half, as shown below and at 4:23 in my video. 					<description><![CDATA[<p>University of Virginia School of Engineering and Applied Science via News Medical &#8211; In the early days of the COVID-19 pandemic, doctors in Wuhan noticed something surprising. Many of the elderly patients who survived the virus were poor: not exactly the demographic you would expect to fare well in a health crisis. A review of the survivors&#8217; medical records revealed that a significant number suffered from chronic heartburn and were taking an inexpensive drug called famotidine, the key ingredient in Pepcid. (Wealthier patients tended to take the more costly drug omeprazole, found in Prilosec.) Was an over-the-counter acid suppressant helping people survive COVID-19? This is how many medical studies begin, said biomedical engineering professor Phil Bourne, who serves as founding dean for the School of Data Science. &#8220;There&#8217;s often a phenomenon that doctors report anecdotally, or that&#8217;s mentioned in passing in a particular research paper, and that provides a clue -; a hook,&#8221; he said. Typically, to find out whether a drug is effective in treating a particular medical condition, scientists develop prospective clinical trials. But this method is expensive and can take years, Bourne pointed out. When faced with a global pandemic, it&#8217;s helpful to explore other options. That&#8217;s where data scientists come in. Bourne and UVA senior scientist Cameron Mura worked with an international team of researchers to analyze information from a database that holds the medical records of millions of COVID-19 patients living in 30 different countries. The team winnowed that number down to around 22,000 people, the largest sample size for a study on famotidine and the disease to date. &#8220;The power of the electronic health record, which is really yet to be fully realized as a research tool, is that you&#8217;ve suddenly got all this data you can mine to see whether what you determined in passing or anecdotally has any basis.&#8221; Phil Bourne, Biomedical Engineering Professor The team&#8217;s analysis, which appeared in the journal Signal Transduction &#38; Targeted Therapy(from the Nature publishing group), showed that the data supported findings from other smaller-scale studies. When delivered at high doses (the equivalent of about 10 Pepcid tablets), famotidine appears to improve the odds of survival for COVID-19 patients, especially when it is combined with aspirin. It also seems to hinder the severity of disease progression, making patients less likely to reach the point where they require intubation or a ventilator. The next challenge was to figure out why. Data scientists like Mura and Bourne perform extensive detective work for medical analyses like this one, looking at existing information and drawing upon biochemical and molecular principles to propose a cohesive theory that helps elucidate the population-scale patterns they identify. Mura calls this &#8220;weaving a story&#8221; based on the data. He needed to work backwards from massive groups of people and draw some possible conclusions about what was happening at a totally different scale -; the scale of proteins that are &#8220;one millionth the size of an ant,&#8221; he said. One of the most dangerous phenomena COVID-19 can trigger in your body is something called a cytokine storm, which is a potentially fatal amplification of an immune response. When you become sick, your immune system releases inflammatory proteins called cytokines that tell your immune cells how to fight the infection. But in more severe illnesses, cytokine production can spiral out of control, becoming dysregulated. &#8220;Basically, your immune system goes haywire and starts attacking things like your otherwise healthy lung tissue because it&#8217;s so desperate to kill off the invading virus,&#8221; Mura said. &#8220;Your own physiology essentially uses a sledgehammer against the pathogen when a fly swatter would suffice.&#8221; The team&#8217;s theory is that famotidine suppresses that reaction. Although it was developed with a specific purpose in mind -; blocking the histamine receptors that help produce acid in your stomach -; famotidine, like all other medications, can cause side effects. Mura and his colleagues believe that interfering with cytokine storms might be one of them. &#8220;It may well be a case of famotidine having a beneficial off-target effect,&#8221; Mura said. We generally think of side effects as a bad thing, but in some cases, they can be harnessed to treat other conditions. In the future, it&#8217;s possible that famotidine could be re-purposed in this way. But the team&#8217;s findings are far from conclusive. Other studies have offered conflicting pictures of what famotidine can do for COVID-19 patients: Some have found that it has a neutral effect and one has even suggested that it might be detrimental. Mura, Bourne and their colleagues recently published a review of existing research on the subject, along with suggestions for a framework that could help reconcile the contradictory reports. Still, with its unique focus on combining famotidine with aspirin and its impressively large sample size, the team&#8217;s study has shed further light on an inexpensive and safe potential treatment that would be easy for doctors to prescribe. In the midst of an international health crisis, the study has also laid important groundwork for further research. &#8220;Scientific studies are sometimes viewed as the end-all, be-all, but they&#8217;re really just a starting point or a springboard,&#8221; Mura said. &#8220;Any good study raises more questions than it answers, and data science is often what kick-starts that process.&#8221; To read the original article click here.</p>
